You are the Customer identified in the Online Sign Up Form(if the identified Customer is a legal entity, you are legally entitled to act on its behalf). You can use our website to perform any or all of the following:
Place orders with us
Obtain information about orders you have previously placed with us
Manage your account with us
Download literature and other information about our products
Subscribe and unsubscribe to email notifications and confirmations
Perform many other tasks
If you choose to use this website, you are undertaking obligations to us and we are undertaking obligations to you. We want you to understand these obligations. They are set out in this Agreement.

1.Privacy and Confidentiality.
a. Our website is a secured site that is also password-protected. When you accept this Agreement you will be provided with at least one login name and at least one password. (You may be provided with more than one login name and password, if you request this and we agree with your request.) You agree that any communication sent to our website using one of your login names and a corresponding password will bind you as if you had sent it. You are responsible for protecting your login name(s) and password(s).
b. We agree to maintain in confidence all patient information you provide to us at our website. You agree that you will not provide any patient information to us unless you are authorized to do so.
c. We agree to maintain in confidence all business information you provide to us at our website, and to use such business information only for purposes of our business relationship with you.
d. You agree to keep in confidence any information you learn at our website, and to use such information only for purposes of your business relationship with us.

Any unauthorized use of the Content may violate copyright laws, trademark laws, the laws of privacy and publicity, and civil and criminal statutes.
Except as provided in this Agreement, you are prohibited from copying, reproducing, modifying, distributing, displaying, performing or transmitting any of the Contents of the MySiemens site for any purposes, and nothing otherwise stated or implied in the MySiemens site confers on you any license or right to do so.
Permission is granted to use the Contents contained in the Library section of the MySiemens site solely for print reproduction only. You may download one (1) copy only of the Content to be used only by you and maintained on a single non-networked computer. Any other use, including electronic distribution or posting of the Content on other web sites, email communications, network computers, broadcast or other electronic media, is strictly prohibited without our express prior written consent. 6.Administrator and User Access.
By signing up online, you are automatically designated as the Administrator of the account. You can also be granted Administrator privileges by signing up via your SHI Sales Representative. The Administrator shall designate those individuals who have the authority to initiate transactions or obtain information pertaining to your Account ("Users"). You agree that such designation by the Administrator shall be considered proper authorization of a User by the Customer and that such authority shall continue to be effective until and unless the Customer gives written notification to SHI that such authority has been revoked or the Administrator removes the individual as a User from the "Manage User Access" section of mySiemens and SHI has had sufficient time to process such revocation.

7.Authorization.
The person who makes this Agreement on behalf of the Customer identified in this agreement and by the account number asscociated with this agreement represents and warrants that he/she is authorized to make this Agreement on behalf of the Customer and to use our https://www.usa.siemens.com/hearing website on behalf of the Customer.

IN WITNESS WHEREOF, Customer and SHI have caused this Agreement to be made and delivered by their duly authorized representatives, all as of the date shown below.
